Role Summary

Our Registered Nurse (RN) serves a pivotal role in our efforts to provide compassionate, top-notch care for our patients in the comfort of their place of residence. By personifying our Mission and Values, our Registered Nurse (RN) acts as a bridge to independent living for our clients who may also require nursing care, under the supervision of a registered nurse, coordinate care with the interdisciplinary team, clients and their families, and the referring Agency.


Essential Functions:

  1. Assist clients with tasks within the scope of a home health aide based on the Plan of Care (POC).
  2. Communicate effectively with administrative staff regarding client/family needs assessments, including reporting any significant changes in client condition.
  3. Provide care as outlined in the Home Health Aide Care Plan and accurately document the cares provided.
  4. Contribute to continuity of care, effective communication between all care team members, and regular participation in Case conferences/Team Meetings.
  5. Apply concepts of infection control and universal precaution in coordination/performing client care activities to protect both staff and patients.
  6. Demonstrate commitment and professional growth by participating in in-service programs and maintaining/improving competency. Completes training or in-services in a timely manner as assigned by your supervisor.
  7. Meet mandatory continuing education requirements of the Agency/licensing board.

Requirements

Experience:

  • (1) year experience as a home health aide or certified nursing assistant in a home health setting. Successful completion of formal training in nursing arts may be substituted for the qualifying experience, subject to applicable State law, regulation, or rules.

Skills:

  • Ability to establish and maintain effective working relationships with all segments of the staff, and the Agency leadership team.
  • Shall be able to read, write and comprehend English.

Education:

  • High School Graduate or equivalent
