Home Health Physical Therapist (PRN)

As a Home Health Physical Therapist, you will play a crucial role in assessing and evaluating the physical therapy needs of our clients and developing personalized care plans to improve their mobility, strength, and overall well-being. Responsibilities

• Conduct comprehensive physical assessments to identify clients' needs and develop appropriate care plans
• Provide physical therapy services to clients in their homes, including therapeutic exercises, manual therapy, and assistive device training
• Collaborate with interdisciplinary teams to ensure coordinated and seamless care delivery
• Educate clients and their families on injury prevention, self-management techniques, and home exercise programs
• Monitor clients' progress, document observations, and adjust care plans as needed
• Adhere to all legal and ethical guidelines, maintaining confidentiality and ensuring the highest level of professionalism
• Maintain accurate and timely documentation of client evaluations, interventions, and progress notes

Requirements

• Degree in Physical Therapy from an accredited university
• Valid Physical Therapy license in the state of Michigan
• Minimum of 1 year of experience as a Physical Therapist in a home healthcare setting is preferred, but not required
• Strong communication and interpersonal skills to effectively collaborate with clients, families, and healthcare teams
• Ability to work independently and prioritize tasks
• Proficient in electronic medical record systems and other relevant software applications
