POSITION:  SPECIFIC ACCOUNTABILITIES

PROFESSIONAL PRACTICE

  • Utilizes nursing skills to assess patient and family needs and develop a palliative plan of care.
  • Communicates plan of care with all care providers working with patient and family.
  • Collaborates with physicians to ensure successful medical management of distressful symptoms.
  • Assists in development of the care plan in conjunction with the attending physician, case managers, nurses, chaplains, dietary, Massage Therapists and other health care professionals.
  • Leads weekly interdisciplinary meetings to facilitate continuity of care by disciplines involved in Palliative Care.
  • Coordinates and ensures continuity of palliative care by health care providers in the various settings including long term care, home care and hospice.
  • Documents all team interventions according to JCAHO standards and in compliance with professional practice standards.
  • Acts as advocate for patient and family wishes within the healthcare system.
  • Serves as preceptor for new palliative care nurses and for ANGEL nurses at site(s) where s/he practices.
  • Applies legal and ethical principles in the analysis of complex issues in end of life care, recognizing the influence of personal values, professional codes and patient preferences.
  • Communicates effectively and compassionately with patient, family and interdisciplinary team about end of life issues.
  • Develops partnerships with key leadership stakeholders, physicians, department managers and targeted community resources/agencies that promote palliative care quality and process improvements for patients and families.
  • Maintains clinical database of patients and families served through palliative care teams.

EDUCATIONAL PROGRAMMING

  • Participates in community education around end-of-life needs and issues.
  • Informally educates customers, providers and other departments within the network on complex care procedures, pain and symptom management

PROFESSIONAL DEVELOPMENT

·Maintains professional accountability through ongoing education and peer collaboration.

  • Pursues and maintains professional certification through National Hospice and Palliative Nurse Association.

EDUCATION, SPECIALIZED AND/OR TECHNICAL KNOWLEDGE REQUIREMENTS:

Active licensure as a Registered Nurse in the state of Ohio. Knowledge of current palliative therapies and interventions. Minimum of three years working with people in the later phases of an active disease process. Master’s Degree in Nursing preferred. Highly motivated self-starter. Excellent verbal and written communication skills.
