MIND & MOBILITY is celebrating 10 years in Florida. We are looking for Florida Licensed Physical Therapist to provide in home therapy for our clients in and around Port St Lucie.

Job Duties:

Candidate will be responsible for in-home therapy visits

With each appointment, evaluate the level of function and help develop/update the care plan

Maintain a positive and joyful attitude at all times, respecting the needs of each individual patient while providing personalized treatment

Partner with family members and other members of the healthcare team to promote the patient's progress toward mutually agreed upon goals

Ability to use an EMR system to write and submit proper progress reports. We have a web-based software for your notes called Optimis PT

Requirements

Requirements:

Current, valid Florida PT license

Valid driver's license

Able to produce all credentials prior to seeing first patient

Level 2 Background Screening w/prints or able to pass one

Able to work a Flexible schedule

Computer literate

Great organization skills and ability to write detailed reports using EMR software

One year as a licensed therapist working with geriatric patients preferred
