Clinic Nurse Navigator - University Pulmonary & Critical Care

University Physicians Association is seeking qualified applicants for a Clinic Nurse Navigator at University Pulmonary & Critical Care located within the UT Medical Center. Candidates must be detail-oriented and able to work under deadlines in a very fast-paced office. Applicants must be familiar with assisting the physician(s) with the assessment, treatment, and coordination of care for patients with Pulmonary Hypertension and other pulmonary conditions in the office/clinical setting. 

GENERAL SUMMARY OF DUTIES: Responsible for assisting the physician(s) with the assessment, treatment, and coordination of care for patients with pulmonary conditions in the office/clinical setting.

TYPICAL WORKING CONDITIONS: Normal clinical office environment. Monday through Friday, No weekends or holidays. 

CLINIC NURSE Navigator DUTIES (but not limited to):

1) Coordinate outpatient clinic processes.

2) Develop/implement protocols.

3) Coordinate prescription management. 

4) Coordinate/facilitate patient transition from inpatient to outpatient. 

5) Coordinate nursing/staff education outpatient/inpatient)

6) Make sure all testing and records are available before patient’s visit.

PERFORMANCE REQUIREMENT: (Knowledge, Skills, and Abilities)

· Knowledge and ability to perform basic clinical skills- phlebotomy, injections, IV therapy, various testing as needed.

· Ability to adjust work pace to facilitate clinic flow. 

· Skill in verbal communication.

· Ability to apply policies and procedures to daily scenarios and deal with a variety of challenges. 

· Ability to work effectively with physician(s), patients, staff and the public.

Requirements

EDUCATION: Minimum associate-level RN degree.

EXPERIENCE: At least six months of experience in a similar clinical setting is preferred. 

CERTIFICATION / LICENSE: Current Registered Nurse (RN).
