<p><span>The Case Manager II assesses client needs, abilities and strengths; facilitates development of client case plans and monitoring to determine progress both individually and as a program. Networks with other agencies to identify and collaborate with resource development and delivery. </span></p>

<p><span><strong><span>About the role:</span></strong></span></p>
<ul>
<li><span>Conducts casework interviews with clients to obtain information for formulating program/service eligibility and case status.</span></li>
<li><span>Identifies psychosocial, economic, and physical needs of clients; assesses and reassesses client's support systems, available community resources and other factors to plan, develop, and implement an appropriate service plan.</span></li>
<li><span>Assist client with housing search, placement, and stability activities.</span></li>
<li><span>Develops and implements a case plan that presents intervention strategies to meet client.</span></li>
<li><span>Refers clients to appropriate community resources and advocates for or assists the client in obtaining such services.</span></li>
<li><span>Prepares complete and accurate case notes; writes correspondence, reports, and other written materials; inputs data into HMIS.</span></li>
<li><span>Reviews all assigned cases on a scheduled basis to determine continuing need for services and participate in case consultations.</span></li>
<li><span>Provides training to new case managers.</span></li>
<li><span>Regularly peer review and audit of case files to ensure of services standards are met and contract compliance.</span></li>
<li><span>Assists in transporting clients and advocating for them with public and private agencies.</span></li>
<li><span>Attends meetings with other community service providers on behalf of The Salvation Army, as assigned.</span></li>
<li><span>Conduct routine home visits based on program and contract requirements. Accompany peers on home visits, as needed.</span></li>
</ul>
<p><span><u>Education:</u> Bachelor’s in social work, human services, psychology, sociology, or criminal justice. </span></p>
<p><span><u>Experience:</u> Three years social work experience. Management experience helpful.</span></p>
<p><span><u>Certifications:</u> Valid Wisconsin Driver’s license with approval to drive from The Salvation Army’s insurance. </span></p>
