About the role

The Care Coordinator RN will be responsible for triaging and assessing care for virtual patient care interactions. You will have a critical role in providing support and education for patients undergoing varied care and treatment plans, in collaboration with a physician/NP provider team. You will serve as an escalation point, providing medical guidance, support, and coaching patients towards improved health outcomes.

Responsibilities

  • Provide patient-focused care and guidance via the LifeMD platform, assessing and addressing medical needs of patients by responding to live electronic portal messaging, phone, and email from patients with clinical questions, concerns, or presentation of symptoms
  • Coordinate lab orders and labs review, prescription orders, radiology tests, and other diagnostic aspects of patient care
  • Serve as a resource to the non-clinical Care Team in dealing with day-to-day clinical questions and concerns
  • Document all patient/member interactions via management software
  • Educate patients/callers with ability to ask open-ended questions and uncover information to make appropriate clinical recommendations
  • Collaborate across the Clinical team on optimizing patient experience, enhancing provider care, and ensuring adherence to regulatory and compliance requirements
  • Collaborate with clinical operations on standard responses as well as new trends that can be triaged with proactive care and improved processes
  • Complies with federal, state, and local legal and professional requirements by studying existing and new legislation, as well as internal LifeMD policies and procedures

Requirements

  • Bachelor’s Degree in Nursing (BSN) from an accredited Nursing Program + Active Registered Nurse licensure with multi-state practice privileges in all 50 states
  • 4+ years experience in Nursing, 2+ years in a telemedicine a plus
  • Communicates clearly and effectively with patients and other team members, peers and supervisors, verbally and in writing
  • Comfortable with technology such as teleconferencing and electronic charting
  • Flexible and fast learner, comfortable in a fast-paced and changing environment
  • Excellent organizational skills and an aptitude for detail
  • Kind, empathetic, positive, and energetic patient interactions
