Job Description
We are seeking a dynamic and compassionate individual to join our team as the Wellness Center Case Manager/Coordinator, a newly established position aimed at connecting our most at-risk students to essential community resources in order to help them experience increased academic success and achieve positive wellness outcomes. The successful candidate will have a Bachelor's degree in social work or a related field, showcasing a strong entrepreneurial spirit and the ability to build relationships, network effectively, and make valuable community connections.
Position Type: Full-Time, Exempt, 10-months, Grant-funded position (two years)
Tentative Start Date: July 22, 2024
This position is grant-funded and is available until the end of the 2025-2026 school year
On a day-to-day basis, you can look forward to the following:
Resource Identification:
- Research and identify relevant community resources within the St. Louis area that can benefit students and their families.
- Establish and maintain strong partnerships with local agencies and organizations to enhance the support network available to our students.
Family Engagement:
- Conduct regular meetings with students and their families to assess needs and ensure they are connected with appropriate community resources.
- Facilitate communication between families and external agencies to streamline the process of accessing essential services.
- Conduct occasional home visits (always in pairs and with no mandatory home entry; the goal is to use this strategy as needed, to support families in connecting with community agencies.)
Program Evaluation and Data-Tracking:
- Implement program evaluation strategies to assess the effectiveness of Wellness Center initiatives and interventions.
- Track and analyze data to ensure compliance with grant guidelines and optimize the efficient use of funds.
Survey Coordination:
- Administer and analyze monthly family and student needs surveys to gather valuable feedback.
- Utilize survey data to identify trends, areas of improvement, and inform the development of targeted Wellness Center initiatives.
Community Engagement Events:
- Collaborate with the Manager of Mental Health to organize quarterly family resources and community events.
- Utilize data gathered from surveys to tailor events that address specific needs and concerns within the community.
Qualifications
What you bring to the table:
- Bachelor's degree in social work or a related field.
- Proven experience in building and maintaining community relationships.
- Strong entrepreneurial spirit and a proactive approach to problem-solving.
- Excellent communication and interpersonal skills.
- Ability to work independently and collaboratively in a team.
- Familiarity with program evaluation and data tracking.
- Willingness to occasionally conduct home visits (training provided).
- Commitment to the well-being of our student population.
- This position will follow the school calendar (including all school breaks and holidays) but have slightly different working hours than our schools, to best meet the needs of families:
- Monday - Friday, 10 am-6:30 pm; on days when there is a family engagement event, your hours will be flexible to accommodate the later evening
