Provides individual attention to each patient and needed support by navigating patients through the complexities of the screening, diagnostic, treatment, and maintenance care continuums.Job Description
Primary Duties & Responsibilities:
- Serves as a resource to assist patients and family navigate care continuum, barriers to care, and identifies resources/interventions related to social determinants of health (SDOH).
- Tracks interventions and outcomes.
- Completes and processes all necessary paperwork and documentation to enroll patients in appropriate programs.
- Facilitates communication and education with heath care staff and providers.
- Performs other duties as assigned.

Required Qualifications:
- Basic Life Support certification must be obtained within one month of hire date.
- Basic Life Support certification (Online BLS certifications, those without a skills assessment component, are not sufficient to meet the BLS requirements).
- Oncology Patient Navigator Training: The Fundamentals from George Washington School of Medicine and Health Sciences within 45 days of hire.
- Leadership in Oncology Navigation American Cancer Society within six months of hire.
