Position Summary

Provides individual attention to each patient and needed support by navigating patients through the complexities of the screening, diagnostic, treatment, and maintenance care continuums.

Job Description

Primary Duties & Responsibilities:

  • Serves as a resource to assist patients and family navigate care continuum, barriers to care, and identifies resources/interventions related to social determinants of health (SDOH).
  • Tracks interventions and outcomes.
  • Completes and processes all necessary paperwork and documentation to enroll patients in appropriate programs.
  • Facilitates communication and education with heath care staff and providers.
  • Performs other duties as assigned.

Required Qualifications

Required Qualifications:

  • Basic Life Support certification must be obtained within one month of hire date.
  • Basic Life Support certification (Online BLS certifications, those without a skills assessment component, are not sufficient to meet the BLS requirements).
  • ​Oncology Patient Navigator Training: The Fundamentals from George Washington School of Medicine and Health Sciences within 45 days of hire.
  • Leadership in Oncology Navigation American Cancer Society within six months of hire.

Salary Range

$19.21 - $28.85 / Hourly
