OPS Family Support Worker
Hourly Rate $18.00 per Hour approximately 25 hours a week.
The OPS Family Support Worker contacts pregnant women, interconception women and families of children under the age of three to assess risk factors that may lead to poor pregnancy outcomes and/or poor developmental outcomes. The position works in partnership with the client or family and provides information and linkage to programs and resources to address risk factors identified by a risk screen or referral. The OPS Family Support Worker will serve a critical role in connecting with pregnant women, and families with newborns, to home visiting services and supports. Using motivational interviewing techniques, the OPS Family Support Worker conducts initial client intakes, ensuring a supportive and personalized approach. All services provided are documented in the programs data system.
Ensures that CI&R (Coordinated Intake and Referral) performance benchmarks are completed as outlined in the Healthy Start Standard and Guidelines and the agencies internal policies and procedures
Conducts Initial Intakes, including attempts to contact, to assess referred women and infants needs and risks. Travel to attempt to contact clients in the service area may be required
Works cooperatively with Healthy Start staff members and community partners to ensure each potential home-visiting client is provided with knowledge and resources that best suit their individual needs
Explains home visiting options to potential clients and provides a referral to the appropriate home visiting service provider
Follows up to ensure referral is received by service provider and services are provided to families
Addresses questions and concerns and provides contact information for helpful community resources
Provides relevant information to clients about breastfeeding, health care options, parenting education, preventative health education and child development
Completes data entry of participants' sensitive information and treats this information within HIPAA privacy rules. Will maintain confidentiality of children, families and other records and information at all times.
Ensures that all required documentation is typed and submitted in a timely manner into the Well Family System and maintains the participants record in accordance with program requirements
Works closely with the Senior Human Services Program Coordinator to meet required programmatic benchmarks
Participate in trainings, meetings, workshops, conferences, webinars, and conference calls as necessary
Performs other duties as assigned
