Job Summary:
The Primary Care Nurse Practitioner will provide comprehensive healthcare services in an outpatient setting. This role involves assessing, diagnosing, and managing a wide range of health conditions for patients across all age groups. The ideal candidate will be committed to delivering exceptional care, focusing on preventive measures and patient education.
Responsibilities:
- Conduct comprehensive patient assessments, including physical examinations and health history evaluations.
- Diagnose and manage acute and chronic illnesses, coordinating care plans as necessary.
- Develop and implement individualized treatment plans based on patient needs and evidence-based practices.
- Prescribe medications, therapies, and other interventions in accordance with clinical guidelines and state regulations.
- Provide patient education on health promotion, disease prevention, and management of chronic conditions.
- Perform routine procedures (e.g., immunizations, screenings, minor office procedures) in a safe and efficient manner.
- Collaborate with physicians, specialists, and other healthcare providers to ensure continuity of care.
- Maintain accurate and confidential electronic medical records, ensuring compliance with regulatory standards.
- Stay current with developments in primary care and participate in continuing education opportunities.
- May travel to other CLS clinics as needed.
- Performs other related duties as assigned.

Requirements
- Current state licensure as a Nurse Practitioner (NP).
- Previous experience in an outpatient primary care setting preferred.
- Ability to work collaboratively in a multidisciplinary team.
- Strong communication and interpersonal skills, with a focus on patient education and empowerment.
