What You’ll Do:

• Deliver comprehensive in-home primary care for seniors
• Diagnose, manage, and treat acute and chronic conditions
• Prescribe medications and create care plans
• Typically see 1–2 patients per hour to allow for quality time and relationship-building.
• Collaborate closely across care teams and document using EMR
• Partner with support staff for prescriptions, labs, and overall care coordination

Required Qualifications:

• Nurse Practitioner license (Georgia), DEA, board certification
• Valid driver’s license, clean driving record, and auto insurance
• Master’s degree and BLS (AHA or ARC)
• Strong tech skills and comfort with laptops/tablets
• A genuine passion for advancing patient care and experience

Preferred Skills:

• At least 1 year of primary care experience (geriatrics preferred)
• Experience with primary care house calls
• Familiarity with value-based care, HEDIS metrics

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.

 

$115,200 - $158,400 per year

 

This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
