We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team at a group home serving individuals with intellectual and developmental disabilities. The primary responsibility of this role is to provide direct care including G-tube feeding, support with Activities of Daily Living (ADLs) and overall health monitoring to ensure the safety and well-being of our individuals.
Responsibilities:
- Administer G-tube feedings in accordance with prescribed care plans
- Assist individuals with ADLs including hygiene, mobility, and personal care
- Ensure accurate documentation in compliance with NJ DDD
- Assist with other household duties such as cooking, cleaning, and community outings as part of a team-oriented approach
Qualifications:
- Graduate of an accredited nursing program
- Valid LPN license in the State of New Jersey
- Proficient in G-tube administration and related care
- Valid NJ Driver’s License with a clean driving record
- CPR/First Aid certification required
- Experience in community health or home care settings preferred
Shifts available: Monday to Friday 4pm-8pm, Saturday and Sunday 9am-1pm and 4pm-8pm.
This position is based in Teaneck NJ and can be filled as part time or full time.
