Job Description

Be a part of a world-class academic healthcare system, University of Uchicago Medicine, as a Nurse Navigator in the Heart and Vascular department.

 

As Nurse Navigator, you will be responsible for coordinating the care of patients through the disease process. This specific position will support the Bloodless program. 

 

In this role, the Nurse Navigator acts as a clinical liaison for patients, across medical services and clinical departments. The Nurse Navigator assess clinical, emotional, spiritual, psychosocial, financial, and other patient needs. This position will also evaluate and manage patient symptoms before during and after treatment.

Essential Job Functions

  • Acts as liaison for patients, families and their multidisciplinary clinical team.
  • Facilitates resolution to patient needs and monitors follow up.
  • Educates/reinforces education with patients and families on disease, treatments, side effects, adverse reactions, and reportable signs/symptoms based on multidisciplinary care plan.
  • Develops and/or coordinates development of patient education materials, clinical pathways, and quality studies.
  • Coordinates all new patient appointments and facilitates record gathering process.
  • Serves as primary point of contact for patients.
  • Serves as call center contact.
  • May be responsible for direct patient care.
  • Performs other duties assigned.

 

Required Qualifications

  • Graduate of an NLN accredited School of Nursing
  • Associate Degree in Nursing
      • Associate Degree Nurses are required to enroll in a BSN program within six (6) to nine (9) months of hire, and make progress toward and complete their BSN degree within 30 months. ADN nurses receive 100% tuition reimbursement towards a BSN degree from an accredited program
  • Current Illinois RN license
  • Active BLS certification from the American Heart Association (AHA)
  • Must have general computer skills and be adaptable to changing working situations and work assignments.
  • Excellent interpersonal skills required, with specific ability to communicate effectively with patients, family members, and physicians.

Compensation & Benefits Overview

 

UChicago Medicine is committed to transparency in compensation and benefits.  The pay range provided reflects the anticipated wage or salary reasonably expected to be offered for the position.

 

The pay range is based on a full-time equivalent (1.0 FTE) and is reflective of current market data, reviewed on an annual basis.
