Cleveland Clinic’s Social Work team provides caring, patient and family-focused social work services to meet basic human needs, enable clients to reach their optimal level of psychosocial functioning and enhance well-being. In this role, you will coordinate the transition of care to help patients move through the hospital and ensure that they have everything they need to make the transition home. Here, you will experience various educational opportunities and the ability to earn certifications within the field. 

A caregiver in this position works days from 7:00AM – 3:30PM, including rotating weekends and holidays. 

A caregiver who excels in this role will:  

  • Assist patients and families in achieving improved health outcomes by reducing the bio psychosocial spiritual obstacles that interfere with their ability to use health resources effectively.  

  • Assess patient and family's psychosocial risk factors through evaluation of prior functioning levels, appropriateness and adequacy of support systems, reaction to illness and ability to cope. 

  • Assess patient and family's financial, social, emotional, educational and environmental resources and identify needs based on patient's current medical condition. 

  • Develop an individualized plan of care based on the findings of the assessment and needs of the patient and family/caregiver as related to the patient's medical condition. 

  • Participate in discharge planning activities for complex patients to ensure a timely discharge and to provide appropriate linkage with post-discharge care providers. 

Minimum qualifications for the ideal future caregiver include:  

  • Bachelor's Degree in Social Work (BSW) from an accredited school of Social Work
