Overview

This position is 7on/7off with On Call

 

The Registered Nurse (RN) will be responsible for providing care and services for hospice patients where they reside. This position will be after hours on-call working seven nights/ full weekend on and seven nights/full weekend off.

Responsibilities

  • Performs assessments of patient/family physical, emotional, psychosocial, spiritual, and culture needs to ensure and coordinate the delivery of comprehensive, comfort-focused services through the development of the Hospice patient plan of care. 
  • Provides a full range of professional nursing care to patients via phone or in their homes, under the medical direction of a licensed physician. Maintains accurate, up-to-date records of patients and care given.
  • Documents all patient/family interactions in the medical record per agency policies/protocols. 
  • Works well within a team environment to collaborate with nurses, social workers, chaplains, volunteers, and LNA’s.
  • Can work independently and on your own from a day-to-day basis with activities which include: phone, email and written communication, meetings with a variety of healthcare professionals, and an understanding of hospice rules and regulations.
  • Regularly reevaluates patients' nursing needs and informs physician and other personnel of any changes in patient's condition or needs. 
  • Responds to overnight calls for designated needs (secondary triage; initial triage handled by third-party administrator).

    • Timeliness of clinician documentation (within 24hours)
    • Meets productivity standards
    • Maintains compliance with HOPE visits

    Qualifications

    REQUIRED QUALIFICATIONS

     

    • Must be a licensed Registered Nurse in Vermont and New Hampshire or hold a compact license.
    • Graduate of an accredited school of nursing

     

    PREFERRED QUALIFICATIONS

     

    • Minimum two years’ general experience in a supervised nursing setting recommended. 
    • Hospice/Homecare experience preferred.
    • Certified in Hospice/Palliative Care Nursing desirable.

    Required Licensure/Certifications

    • Unencumbered RN license in VT and NH or compact.
