Registered Nurse Hospice

Overview

This position is 7on/7off with On Call

 

The Registered Nurse (RN) will be responsible for providing care and services for hospice patients where they reside. This position will be after hours on-call working seven nights/ full weekend on and seven nights/full weekend off.

Responsibilities

Performs assessments of patient/family physical, emotional, psychosocial, spiritual, and culture needs to ensure and coordinate the delivery of comprehensive, comfort-focused services through the development of the Hospice patient plan of care. 

Provides a full range of professional nursing care to patients via phone or in their homes, under the medical direction of a licensed physician. Maintains accurate, up-to-date records of patients and care given.

Documents all patient/family interactions in the medical record per agency policies/protocols. 

Works well within a team environment to collaborate with nurses, social workers, chaplains, volunteers, and LNA’s.

Can work independently and on your own from a day-to-day basis with activities which include: phone, email and written communication, meetings with a variety of healthcare professionals, and an understanding of hospice rules and regulations.

Regularly reevaluates patients' nursing needs and informs physician and other personnel of any changes in patient's condition or needs. 

Responds to overnight calls for designated needs (secondary triage; initial triage handled by third-party administrator).

• Maintains compliance with HOPE visits

Qualifications

REQUIRED QUALIFICATIONS

 

• Must be a licensed Registered Nurse in Vermont and New Hampshire or hold a compact license.
• Graduate of an accredited school of nursing

 

PREFERRED QUALIFICATIONS

 

• Minimum two years’ general experience in a supervised nursing setting recommended. 
• Hospice/Homecare experience preferred.
• Certified in Hospice/Palliative Care Nursing desirable.

Required Licensure/Certifications

• Unencumbered RN license in VT and NH or compact.
