About the role

JOB SUMMARY:  The Oncology Patient Navigator is a member of the Atrium Health Wake Forest Baptist Cancer Center Navigation team. Patient navigators work directly with cancer patients, families, and caregivers to identify and overcome barriers to care and facilitate timely access to healthcare services.

 

EDUCATION/EXPERIENCE:  Bachelor’s degree required.  Bachelor's degree in Social Work, Sociology, Education, or Psychology preferred. 

 

LICENSURE, CERTIFICATION, and/or REGISTRATION: N/A       

 

ESSENTIAL FUNCTIONS:

  1. Communicate effectively with patients, families and caregivers while demonstrating empathy, compassion, honesty, and cultural sensitivity.
  2. Identify and address barriers to care by connecting patient and their families with appropriate resources within the heath system and community.
  3. Collaborates with other healthcare professionals and the patient’s care team to facilitate timely access to care and resources.
  4. Documents accurately and completely in EMR concerns patient encounters (i.e., barriers identified, education provided, resources utilized and any other further interventions and follow up).
  5. Provides patient and family centered care throughout the continuum of care, diagnosis through treatment and transition to survivorship or supportive care.

 

SKILLS/QUALIFICATIONS:

Excellent interpersonal, oral, and written communication skills with ability to interact effectively with patients and their families.

Strong organizational and time management skills.

Ability/temperament to adapt to changing conditions and diverse job tasks.

Demonstrate teamwork and professionalism in the workplace.

Ability to work independently and collaboratively.

Basic computer skills required.

Ability to respond promptly to communications from the healthcare team to deliver quality, timely care to patients.

Knowledge of EPIC electronic medical record preferred.
