Responsibilities & Requirements

The Community Wellness Program Specialist position duties include, but are not limited to the following:

• The Program Specialist reports to the Community Outreach Manager and will assist in maintaining the wellness activities for the Community Outreach Dept.
• This position will support the Program Coordinator's in the development and implementation of wellness programs, provide education and promote the Years Ahead program to increase membership participation.
• The Program Specialist will participate in the day-to-day operations of the Call Center answering multi line phones and is responsible for processing calls including Physician Referral Service.
• Regularly communicate effectively with patients/family/significant others of varying ages and cultural backgrounds.
• Must be willing to transport work related materials, in employee's own personal vehicle, to on and offsite locations as needed.
• Other duties as assigned. 

Job Requirements

• High School diploma required
• CPR certification.
• Experience in Health Care setting.
• Excellent organization, presentation and communications skills required.
• Ability to work varied hours with some evenings and weekends. Highly motivated and able to work independently.
• Computer skills - Microsoft Office Suite.
• Requires the ability to lift 30-50 pounds as needed.

Preferred Qualifications

• Bachelor’s degree in health care, geriatrics, marketing or related field preferred.
• Experience in planning health promotion programs preferred.
