This position can be home-based.

 

This is a full time, benefits eligible, grant funded opportunity. Current funding will expire 6/30/2028.

Responsibilities

The National Sr. Program Implementation Lead will own the exploration and evaluation of innovative products and programs supporting participation in being responsible and accountable for activities and commitments for all assigned Cardio Kidney Metabolic Health (CKMH) initiatives.  Responsible for directing the lifespan of initiatives, including exploring and evaluating innovative products and programs to assist with site participation in CKMH initiatives. Responsible for directing innovation components and initiatives focused on improving the quality of care and treatment for patients of CKMH.

 

This individual will engage in collaborative activities within 15 regions for the CKMH initiative. This individual will identify and oversee the evaluation of data acquisition strategies and evaluate their effectiveness in markets. The Senior Program Implementation Lead will work closely with leadership across all pillars of the CKMH Initiative.

 

The successful candidate should possess a strong understanding of cardiovascular disease, prior experience developing innovative products and services within regional and national quality improvement initiatives, and prior experience developing and supporting QI products and programs with multiple health systems and healthcare settings. The candidate must also have experience working with clinical data from outpatient clinics to support quality improvement initiatives. This role will regularly liaise with volunteer and health system thought leaders and work cross-functionally with the Association’s teams. The desired candidate will have strong communication skills, negotiation skills and be results-driven.

 

  • Develop CKMH Initiative innovation plan, including a timeline and implementation plan
  • Work closely with the Association’s Outpatient Registry team for CKHM Initiative onboarding and ongoing strategy
  • Build strategies and relationships with technology leaders and vendors to expand opportunities for participation in the CKMH initiative. Provides consultation, direction, and support for assigned regional and/or national quality initiatives.
  • Monitors and evaluates progress toward initiative innovation goals and objectives and identifies strategies for addressing gaps
  • Facilitates strategy and data quality meetings in coordination with health care volunteers and staff partners while developing a program sustainability plan for innovative technology components of the funded initiative.
  • Meets regularly with AHA and volunteer leadership groups to report progress, barriers, and solutions to achieving commitments for all assigned initiatives. Act as the liaison to facilitate ongoing communication and implementation of a system of care   Works with national staff partners through ongoing face-to-face meetings to ensure grant deliverables are met according to the timeline.
  • Advance health equity focus of the Association’s quality initiatives and programs by aligning funded project proposals with the latest health equity standards, innovations, and data collection recommendations.
  • Lead efforts to improve communication with stakeholders, volunteers and industry to increase understanding of AHA quality improvement implementation science initiatives. 

Qualifications

  • Bachelor's degree or equivalent work experience.
  • Five (5) - Eight (8) years of experience.
