Primary Duties & Responsibilities:
In either a clinical or a field setting, interviews clients with problems such as personal and family adjustments, finances, employment, food, clothing, housing and physical and mental impairments to determine nature and degree of need.
Gathers information such as medical, psychological and social factors contributing to client's situation; evaluates these together with client's strengths.
Provides limited advice to clients individually, in families or in other small groups, regarding how to meet personal needs and aids clients to improve social functioning and life skills.
Participates in helping clients to modify attitudes and patterns of behavior, by providing training and educational programs that help clients understand the nature and source of personal problems.
Refers clients to community resources and other organizations as indicated. May determine or participate in determination of client eligibility for financial assistance.
Compiles records, prepares reports and develops and/or reviews service plan. Follows up to determine quantity and quality of service provided to the client and status of client's case.
Accesses and records client and community resource information, either on paper or via electronic means, to input and retrieve information.

Required Qualifications:
- Basic Life Support certification must be obtained within one month of hire date.
- Basic Life Support certification (Online BLS certifications, those without a skills assessment component, are not sufficient to meet the BLS requirements).
- Equivalent of a bachelor’s degree in a health-related field, such as social work, counseling, sociology, or psychology. Applicants with bachelor’s degrees in other fields may be considered with sufficient case management experience.
