JOB DESCRIPTION:

SUMMARY OF POSITION

Provide Life Skills and Case Management interventions. Attends meetings, maintains records and provides consultation.  Also maintains productivity and budget requirements. Will supervisor bachelor, associates, and high school degreed staff on the team.   Acts as a liaison while overseeing all services provided by the team.

ESSENTIAL DUTIES & RESPONSIBILITIES

• Supervisory Responsibilities for:  Care Coordinator, Rehabilitation Specialist; Family Support Specialist; Visitation Specialist; System of Care Coordinator; Wraparound Facilitator
• Maintain necessary communications and reporting with referral sources and other involved parties as approved by client.
• Maintain effective working relationships with the clinical team
• Maintain timely/accurate clinical records in accordance with organization policies.
• Maintain effective relationships with Community referral sources
• Participate in the development and implementation of quality clinical practices
• Compliance with annual completion of KPI forms for staff.
• Meet billable hour productivity standards.
• Complete ANSA/CANS assessments and reassessments on time.
• Complete clinical documentation within one business day of the service provided.
• Complete treatment plans and other clinical service authorizations in a timely manner.

KNOWLEDGE, SKILLS & ABILITIES

QUALIFICATIONS

Education Level              

Bachelor’s Degree required, degree in human services field preferred.
