Support the practice by following standards and policies in handling medical records, processing records requests and, when needed, assisting front desk staff.

I. Major Responsibilities:
1. Answer telephones, arrange scheduling, take messages, transfer calls, may call patients to confirm appointments.
2. Coordinate chart function; prepare charts, assure that necessary documents are available.
3. Communicates as needed with physicians and other health care professionals to clarify diagnoses or to obtain additional information.
4. Provides charts/documents requested for use in legal actions, following patient consent /confidentiality protocols.
5. Uses computer programs if requested to tabulate and analyze data.
6. Review and prepare documents to ready them for scanning into EHR.
7. Scan documents into batches in scanning module and sort into appropriate folders in patient’s EHR.
8. Accurately release patient’s health information upon receipt of signed authorization from patient/legal guardian in a timely manner.
9. Accurately maintains paper charts alphabetically.
10. Maintain knowledge of current OSHA and CLIA regulations, and Harrington HealthCare System policies.
11. Assist coworkers as possible to assure smooth office operation and delivery of excellent services through teamwork.
12. Perform other duties as assigned.

II. Position Qualifications:

License/Certification/Education:
Required:
1. High school completion or GED.

Experience/Skills:
Required:
1. Basic computer knowledge.

Preferred:
1. Preferably, some medical records experience in a healthcare setting.  

Unless certification, licensure or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements.

Department-specific competencies and their measurements will be developed and maintained in the individual departments.  The competencies will be maintained and attached to the departmental job description.  Responsible managers will review competencies with position incumbents.
