About the role

Job Summary:

Assist hospice patients with activities of daily living, providing personal care and assist in maintenance of a safe and clean environment under the supervision of the Team Coordinator or Primary Care Nurse. Provides personal care to promote the comfort and well being of the patient including bathing, oral hygiene; hair, nail and skin care, assistance with dressing etc. Provides general care of lifting, turning ambulating patient etc. Must take and record accurate vital signs. May prepare, serve and assist or feed the patients if necessary.

Essential Job Duties:

  1. Performs or assists the patients with activities of daily living using the Relationship Based Care patient delivery model.
  2. Assists the healthcare team treat the patient by collecting, reporting, and documenting patient data.
  3. Maintains high patient and family satisfaction levels by providing care that is based upon the individual needs of the patient and their age/specific developmental needs.
  4. Contributes to a high performance work environment by maintaining specific knowledge and skills needed to perform tasks according to policy and procedure at Good Samaritan Hospital.

Secondary Job Duties That May be Reassigned:

  1. Performs other duties as assigned or requested

Job Specifications:

  • Education: High school diploma or GED is required.
  • CNA required.  Must Maintain current CPR certification, valid driver's license and liability insurance.
  • Experience: Minimum of 6 months experience is preferred.
