Job Description:

A Registered Nurse (RN) in the Referral Coordination Team (RTC) will provide nursing care services in line with established clinical protocols and VA guidelines. This position requires independent judgment, effective collaboration with multi-disciplinary teams, and consistent communication with veterans and care providers. The RN will ensure quality patient care and continuity while adhering to VA standards and regulatory requirements.

Location: Rocky Mountain Regional VA Medical Center (RMRVAMC), 1700 N. Wheeling Street, Aurora, CO 80045.

Key Responsibilities:

1. Direct and Indirect Nursing Care
• Conduct clinical assessments, establish nursing priorities, create care plans, and evaluate outcomes in coordination with interdisciplinary teams.
• Provide high-quality care in accordance with The Joint Commission (TJC), American Hospital Association (AHA), and VA standards of practice.
• Ensure appropriate medical record documentation and secure handling of patient data in compliance with HIPAA and VA regulations.
• Review consults for completeness, appropriateness, and perform utilization reviews.
• Coordinate care with veterans, community providers, internal providers, and administrative staff to ensure timely care delivery.
• Perform clinical reviews of incoming medical records, ensuring effective communication with the ordering provider and Primary Care Providers (PCPs).
• Act as a liaison between the veteran and healthcare teams for proper care management.
2. Documentation and Compliance:
• Complete medical record documentation as per VA Health Information Management standards.
• Adhere to rules and regulations governing VA’s use of Electronic Health Records (EHR), including VISTA and optical disk imaging technology.
• Participate in quality improvement projects and initiatives.
• Follow up on safety reports, including patient safety incidents, medication errors, and other breaches of care.
3. Required Participation and Training:
• Completion of VA-required training and background investigations, including personal identity verification (PIV).
• Attend required staff meetings, including team huddles, RN staff meetings, and annual team training sessions.
• Compliance with required annual cybersecurity and patient privacy training courses.
• Maintain certifications, including ACLS/BLS certifications.

Required Qualifications:

• Education:
• Graduate of a school of professional nursing approved by the appropriate State agency and accredited by an accrediting bodies at the time the program was completed by the applicant.
• Licensure & Certification:
• Active Registered Nurse (RN) licensure: current, full, active and unrestricted registration as a graduate professional nurse in a State, Territory, or Commonwealth (i.e. Puerto Rico) of the U.S. or in the District of Columbia.
• Current ACLS and BLS certification (must be maintained).
• Board certification (e.g., Care Coordination, Nursing).
• NPI number.
• Experience & Skills:
• Minimum of 10 years of nursing experience in clinical practice, with a focus on care coordination or referral
