Bicultural and bilingual in one of the predominant immigrant communities and languages in pediatric primary care (e.g., Somali, Mai Mai, Nepali, Swahili) preferred.
Job Summary:
The developmental support specialist will work with the pediatric primary care population to help complete developmental screening, including screening for autism spectrum disorder, both during and after visits. When developmental concerns are identified, provide support and help connect to needed community resources, such as Help Me Grow Vermont, Early Intervention teams, Autism Assessment programs and direct service providers. They will assume a role with the interdisciplinary teams, including pediatric providers, nursing and care management team to achieve optimal patient-centered, clinical and resource outcomes. They will primarily focus on decreasing existing barriers to screening, diagnosis, and connection around developmental concerns, including language and cultural barriers.
Education:
Associates Degree in Human Services or 2-3 years of experience in a healthcare or community care setting (or equivalent).
Bachelor’s level degree in Human Services preferred.
Experience:
Care Coordination experience preferred.
Customer Service experience preferred.
