About the role

The Pharmacy Documentation & Order Review Coordinator plays a key role in supporting safe, timely, and compliant medication dispensing within Alps LTC Pharmacy. This position is responsible for the ongoing review of active medication orders, management of renewal and documentation requirements, and coordination with prescribers, facility staff, and internal pharmacy teams to ensure uninterrupted therapy and regulatory compliance.

This role requires strong attention to detail, organization, effective communication, and the ability to proactively manage documentation workflows in a fast-paced long-term care environment. This position is a safety sensitive position. 


Qualifications

  • High school diploma or equivalent required
  • Experience in a pharmacy, healthcare, or long-term care setting
  • Familiarity with pharmacy order workflows or documentation requirements
  • Experience with pharmacy management systems or electronic health records (EHR)
  • Understanding of prescription validity, renewals, and compliance standards
  • Strong attention to detail and organizational skills
  • Ability to manage multiple tasks and prioritize effectively
  • Strong written and verbal communication skills
  • Ability to work independently and collaboratively in a team environment
  • Professional and service-oriented communication style


Responsibilities

  • Review active medication orders on an ongoing basis to confirm continued validity, completeness, and readiness for processing

  • Initiate, track, and follow up on medication renewal requests with prescribers to prevent order expiration or dispensing delays

  • Identify orders at risk of interruption and proactively address documentation or clarification needs


  • Obtain missing or incomplete prescription documentation from prescriber offices to ensure orders meet all legal, regulatory, and operational requirements

  • Maintain accurate and up-to-date patient profiles by identifying and correcting missing or outdated required information

  • Maintain prescriber records, including required credentials, contact information, and authorization documentation

  • Document all order-related communications and updates in pharmacy systems to support continuity of care, auditing, and regulatory compliance


  • Communicate with long-term care facility staff to resolve order-related questions that impact medication accuracy or continuation of therapy

  • Coordinate closely with Order Entry, pharmacists, and other internal teams to resolve documentation issues that delay order review or fulfillment

  • Serve as a reliable point of contact for documentation-related inquiries


  • Track outstanding documentation items and follow up to ensure timely resolution and sustained order compliance

  • Identify recurring documentation gaps, workflow inefficiencies, or process issues and escalate them for review and improvement

  • Support quality assurance initiatives related to order documentation and compliance


  • Other Job Duties as assigned
