The Clinical Nurse-Pediatric Complex Care Coordinator serves as a nurse team specialist in the coordination of care for children with medical complexity (CMC) or adults with intellectual/developmental disabilities at an appropriate intensity to accommodate each family’s strengths and needs. Applies nursing scope of practice, and clinical experience and knowledge in the care of children or adults chronic health conditions and disabilities. Participates in the identification of eligible patients within the practice, in order to build a full registry of assigned patients to serve. Leads the practice in obtaining consent from patients or families, in summarizing medical records, in performing a comprehensive child/adult and family assessment and then in developing, sharing and implementing a shared plan of care, with ongoing facilitation of each family’s achievement of their named goals. Works closely within primary care team, including with clinicians, nursing and other team members, to facilitate comprehensive service delivery to the assigned registry of patients, both within and beyond the primary care setting. Seeks both patient/family strengths and barriers to care, including social determinants of health, to assist in the formulation of effective plans of care and achievement of negotiated actions. Assist patients and families with the identification of problems and obstacles to each person’s progressive development towards his/her future goals and develop strategies to mobilize community resources utilizing both formal and informal supports. Anticipate significant transitions and particularly support families through these higher-need periods.
Communicates closely with enrolled patients/families to inform, support, educate and/or intervene in addressing needs to facilitate their receipt of planned, scheduled healthcare and other services to thereby mitigate needs for emergent or crisis care. Utilizes principles of patient-and-family-centered care, cultural humility, trauma-informed practices and motivational interviewing in working with patients and families. Facilitates CMC access to services within health care systems, as well as indicated government and community resources, providing targeted referrals and ongoing communication with these services based on the patient/family’s needs and desired goals. Utilizes self-advocacy organizations, parent to parent networks, diagnosis specific organizations and educational materials as needed. Participates in professional development activities to increase skills to promote the most effective use of available resources, using the training and support team and their resources to assist with knowledge acquisition and future assessment and utilization of new resources. Participates with IC4 training and support team in orientation and training, with ongoing individual coaching and group learning collaborative sessions as scheduled. Collects requisite data for quality improvement assessment of program and family satisfaction with services.
- Requires an Associates of Nursing (ASN); Bachelors of Nursing (BSN) preferred.
