The Manager, Field Care Nursing assesses and evaluates member's needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. The Manager, Field Care Nursing works within specific guidelines and procedures; applies advanced technical knowledge to solve moderately complex problems; receives assignments in the form of objectives and determines approach, resources, schedules and goals.

The Manager, Field Care Nursing employs a variety of strategies, approaches, and techniques to manage a member's physical, environmental, and psycho-social health issues. Identifies and resolves barriers that hinder effective care. Ensures patient is progressing towards desired outcomes by continuously monitoring patient care through assessments and/or evaluations. May create member care plans. Decisions are typically related to resources, approach, and tactical operations for projects and initiatives involving own departmental area. Requires cross departmental collaboration, and conducts briefings and area meetings; maintains frequent contact with other managers across the department.

Required Qualifications Bachelor's Degree 6 or more years of technical 2 or more years of management experience Active, unrestricted Registered Nurse license in the state This role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB. This role is part of Humana's Driver safety program and therefore requires an individual to have a valid state driver's license and proof of personal vehicle liability insurance with at least 100,000/300,000/100,000 limits. Significant previous experience managing cardiology patients in an ambulatory care environment and in an interventional cardiologist cath lab Computer proficiency with knowledge of MS Office products, specifically Word, Excel, and Outlook Experience with electronic health record documentation Must be passionate about contributing to an organization focused on continuously improving consumer experiences Preferred Qualifications Experience in disease management and wellness programs Previous Medicare/Medicaid Experience a plus Knowledge or experience with a patient centered medical home model Experience with eClinicalWorks Bilingual: English, Spanish and other languages Additional Information

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.


 

$86,300 - $118,700 per year


 

This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
