The PUYALLUP TRIBAL HEALTH AUTHORITY, located in Tacoma, Washington is seeking a full-time Clinical Pharmacist (PharmD) to serve as an integral member of the primary care team engaging in direct patient care and collaborative consultations with PTHA's prescribing providers.

CLINICAL PHARMACIST ESSENTIAL JOB FUNCTIONS:

  • Responsible for providing medicine reconciliation recommendations for provider/s patients after reviewing charts, counseling   patients and collaborating with provider.   
  • Responsible for providing chronic care management and chronic non-cancer pain management recommendations for provider/s patients after reviewing charts, counseling patients and collaborating with provider. 
  • Responsible for creating, providing and actively participating in on-site prescription drug education programs to Residents,   Providers and clinical team members as assigned. 
  • Perform pharmaco-therapeutic monitoring of patients in a continuous manner including but not limited to review of progress notes, lab results, prescription refill history, patient interview taking into account age and clinical condition of each patient.
  • Document patient interaction in EHR system on same day.
  • Research drug information to maintain drug knowledge and disease state understanding for application to prior authorization   reviews and ensuring that protocols and guidelines regarding medications are clinically appropriate. 
  • Participate in the development and implementation of medication therapy management programs. (ie: Diabetes Management, Tobacco Cessation, Anticoagulation Clinic).
  • Develop clinical criteria for review of medications, recommend plan design/formulary changes based on best practices, efficacy,   cost, contract availability and third party billing and communicate recommendations to clinical management team. 
  • Counsel patients verbally on the appropriate use of prescription or over-the-counter medications and their contraindications,   storage parameters, dosage, drug interactions, and potential side effects. 
  • Provide education and counseling for patients/clients at approved off site locations.

Requirements

  • Current WA State Pharmacist license required.  
  • PharmD degree in Pharmaceutical Science. 
  • Minimum (10) years outpatient pharmacy experience. 
  • Demonstrable proficiency with computer software programs. 
  • Experience working with electronic records systems.
