The Behavioral Health Consultant, as part of the primary care treatment team, identifies, triages and manages primary care patients with medical and behavioral health problems. This includes providing skill training through psycho-education and patient education strategies and developing specific behavioral/therapeutic treatment plans for patients. The BHC also assists primary health care providers in recognizing and treating mental disorders and psychosocial problems, detecting "at risk" patients, and assessing the clinical status of patients referred by the primary care provider.

Responsibilities
- Assist primary health care providers in recognizing and treating mental disorders and psychosocial problems (Social Determinants of Health).
- Assess the clinical status of patients referred by primary health care providers and triage to the appropriate resource.
- Provide assessment, consultation, and intervention for patients with chronic emotional and/or health disorders.
- Work with primary care team to treat and manage patients with chronic emotional and/or health problems efficiently and effectively.
- Assist with behavioral health diagnosis and reduce stigma by helping to increase primary care provider comfort with behavioral health conditions and associated treatment.
- Assist in the detection of “at risk” patients and development of plans to prevent further psychological or physical deterioration.
- Evaluate patient care plans with primary care team.
- Educate patients, families, and staff about care, prevention, and integrated treatment enhancing techniques.
- Complete timely and appropriate documentation in the Electronic Health Record (EHR) system, including but not limited to, contact logs, progress notes and evaluation notes.
Qualifications
- Master’s degree in Clinical Social Work
- Current Licensed Clinical Social Worker (LCSW) issued by the California Board of Behavioral Sciences
- Valid California Driver’s License, proof of insurance and clean driving record.
- Current Basic Life Support (BLS) Certification for Healthcare Providers
- Computer proficiency, particularly with Electronic Health/Medical Record systems and the Microsoft Office suite, including Outlook, Word, Excel and PowerPoint.
Preferred Qualifications
- Bilingual in English and Spanish (may be required at some clinics).
- At least two (2) years’ experience providing psychotherapy (CBT based models preferred) with children, adults and adolescents.
- Experience providing health psychology services such as smoking cessation, weight management, stress management, substance abuse disorders, or work with chronically medically ill patients.
- A general understanding of trauma-informed care approaches and therapies, Adverse Childhood Experiences and their impact on brain development, suicide prevention and acute crisis assessment/intervention.
- Familiarity or work experience in integrated behavioral health models (IBH).
