- Collaborate with medical and mental health teams to ensure effective patient care and discharge planning.
- Assess psychosocial risks and needs of patients and families, including prior functioning and support systems.
- Counsel patients and families regarding emotional, social, and financial impacts of illness or disability.
- Access and coordinate community and family resources to meet patient needs.
- Provide mental health assessments upon physician referral and deliver therapeutic interventions.
- Support complex discharge planning including guardianship, foster care, adoption, and mental health placements.
- Serve as the primary point person for cases involving child abuse/neglect, adult/elder abuse, institutional abuse, and domestic violence.
- Liaise with protective services, law enforcement, schools, probation, and other agencies.
- Utilize crisis de-escalation, interviewing, and clinical intervention techniques to manage dangerous or high-risk situations.
- Support patients through acute crises, including end-of-life care and guardianship issues.
- Develop and maintain strong working relationships with community organizations and healthcare teams.
- Education: Master’s Degree in Social Work required.
- Licensure: Permanent LSW licensure required; LCSW licensure preferred.
- Certifications: De-escalation certification required within 6 months of hire.
- Experience: 3–5 years preferred in mental health, child and family services, adult protective services, Veteran’s Administration, or substance abuse.
- Additional Skills & Knowledge:
- Knowledge of disease impact and human behavior for assessments and crisis intervention.
- Understanding of legal mandates related to protective services.
- Principles of growth and development across the lifespan.
- Proficiency in crisis de-escalation techniques.
- Ability to develop strong relationships with community agencies, law enforcement, and healthcare teams.
- Proficiency in Microsoft Excel and Word.
- Experience working with diverse patient populations, geographically and financially.
