The Adult Clinical Community Supports (ACCS) Driver promotes the full integration and functioning of adults with mental health services needed to live successfully in the community. The ACCS Driver/Support Staff supports community access for clients by driving their own or company-owned or leased vehicles, transporting and accompanying client to, from and during community appointments as directed by Integrated Team staff.
Job Duties and Responsibilities:
- Promote the safety of clients during transportation.
- Monitor client status, reports concerns or incidents to supervisor.
- Pick up and drop off deliveries of program materials and supplies, as assigned.
- Participate as member of intervention team to perform skills teaching, and help client learn about community resources and obtaining resources.
- Attend medical, psychiatric and other appointments with client as needed.
- Adhere to Medication Administration Program (MAP) policies and procedures.
- Provide crisis intervention to client, as needed.
- Perform other duties as assigned.

The essential job duties/responsibilities of the position include but are not limited to the information listed below:
• Transport and accompany PS to community appointments.• Attend appointments with PS to access, maintain and manage changes to entitlements, benefits, and medications.• Provide transportation to other community outings as directed by Team Leader.• Promote the safety of all PS during transportation.• Arrange routine vehicle maintenance.• Comply with company vehicle procedures.• Develop and maintain service-oriented and professional relationships with community resources and other service providers.• Monitor PS status, reports concerns or incidents to supervisor.• Pick up and drop off deliveries of program materials and supplies, as assigned.• Participate as member of intervention team for assigned PS. At the direction of the Team LPHA Clinician, based on the PS Treatment Plan, perform skills teaching, and help PS learn about community resources and obtaining resources. Provide PS with support and supervision, as needed.• Attend medical, psychiatric and other appointments with PS, as needed.• Adhere to Medication Administration Program (MAP) policies and procedures.• Provide crisis intervention to PS, as needed.• Use trauma informed, recovery-oriented, person-centered, culturally competent and strengths-based approaches s in the work with PS.• Help PS with self-management of symptoms through Safety Plans, and medication administration program services and education, as needed.• Promote engagement, safe housing, community tenure, achievement of greater self-sufficiency and successful completion of service for all PS.• Provide thorough and thoughtful documentation of PS needs and services.• Help with orientation of new staff and PS to the ACCS service.• Attend trainings, as assigned.• Perform other related duties, as assigned.

Qualifications
- High School diploma or equivalent required. Bachelor's degree in human services field is preferred.
- One to two years related experience working with people with psychiatric disability and/or co-occurring disorders preferred.
- Must possess a valid United States Driver's license for at least 1 year.
- Must have access to an operational and insured vehicle and be willing to use it to transport clients
