JOB SCOPE:

 

Under the supervision of the Director of Care Coordination and Health Homes, the Assistant Director supports the oversight of the Care Coordination Department, providing direct clinical and administrative oversight to Program Supervisors and indirect supervision to administrative staff.  In the absence of the Director, the Assistant Director serves as the administrator in charge.   The Assistant Director supports daily operations, including oversight of billing, member crises, quality assurance, and program evaluation including ensuring the programs meet all internal and external regulatory standards and requirements.   

 

 

 

ESSENTIAL FUNCTIONS:

 

  • Provides direct supervision to both the Community and Adult Home Program Supervisors.
  • Provides leadership to all levels of staff on areas of service provision and compliance.
  • Oversees recruitment, hiring and training of care coordination staff.  Evaluates training needs and coordinates using internal and external resources as appropriate to ensure staff has the skills necessary to meet expectations.  Monitors and evaluates staff performance.
  • With the Director, collaboratively establishes performance standards and metrics for all levels of staff.
  • Identifies needs for staff development and coordinates with the Director.
  • Interpret and articulates agency and program mission, policies, practices and procedures for staff.
  • In conjunction with the Director, develops, implements and updates policies and procedures in accordance with regulatory requirements and agency practice.
  • Oversees the timely and accurate submission of weekly billing.
  • Responsible for overall program compliance with regulatory standards.
  • In conjunction with the Director, conducts audit preparation, coordinates program audits and serves as liaison to external regulatory/funders.
  • Conducts quality reviews of cases and oversees crisis management.
  • Utilizes monthly statistical reports that monitor productivity, outcomes, and other critical issues to monitor staff and program performance.
  • Participates in the Agency-wide Incident Review Committee as needed and oversees the reporting of all required reports.
  • Represents PCMH at local and state meetings as required.
  • Performs other duties as may be assigned by the Director / Executive Management.

Qualifications

EDUCATION AND EXPERIENCE:

 

  • Licensed Mental Health Professional (LMHC, LMSW, LCSW)
  • Minimum three (3) years of supervisory experience in administrative/management positions in a mental health/rehabilitation setting with job-related experience providing services to individuals with chronic medical and / or behavioral health conditions.
