This is a remote position.

Position Overview:

As an Outreach and Enrollment Coordinator at Heritage Health Network (HHN), you will play a pivotal role in driving member enrollment and expanding HHN's community presence. You will work closely with healthcare providers, community-based organizations (CBOs), and local stakeholders to engage potential members and facilitate access to healthcare services. This role combines outreach, event coordination, and relationship-building to support HHN’s mission to enhance care for underserved populations.

This role combines remote work and field engagements, with work arrangements tailored to meet program and member needs.

Requirements

Key Responsibilities :

  • Drive Enrollment Growth : Develop and implement targeted outreach strategies to identify and engage potential members, to expand HHN’s membership.

  • Partnership Development : Build and maintain strong relationships with healthcare providers, CBOs, foster agencies, and county facilities to increase referrals and membership opportunities.

  • Community Representation : Represent HHN at community events, health fairs, and meetings, advocating for underserved populations and promoting HHN's programs and services.

  • Sales-Focused Outreach : Engage healthcare providers and facilities by educating them about HHN’s programs, highlighting the benefits for patients and clients to enroll in HHN services.

  • Event Coordination : Organize and lead community health events, informational sessions, and enrollment drives to raise awareness and encourage participation in HHN’s Enhanced Care Management (ECM) program.

  • Feedback Collection : Gather and analyze feedback from stakeholders and community members to refine outreach strategies and enhance enrollment processes.

  • Collaborative Efforts : Work with HHN’s marketing and care management teams to ensure outreach activities align with organizational goals and maximize enrollment.

Qualifications :

  • Education : Bachelor’s degree in Business, Sales, Marketing, or a related field (or equivalent experience).

  • Experience : Proven experience in community outreach, sales, or public health advocacy, ideally within healthcare or social services.

  • Cultural Competence : Understanding of the health needs and cultural dynamics in Riverside and San Bernardino counties, with a focus on underserved populations.
