Schedule: M-F, 8-hour shifts 

 

FlexStaff is seeking an experienced Registered Nurse for a Field RN position for our client, a leader in home health care. This position will cover cases in Manhattan. 

  

This position will manage all aspects of patient care related to services provided in the home. 

  

Responsibilities: 

• Conduct home care assessments 

• Perform home visits 

• Complete all required documentation 

• Communicate effectively with Primary Care Physicians (PCPs) 

• Update medication profiles 

• Coordinate care with other providers 

• Review and complete case communication notes 

• Participate in discharge planning activities 

• Other duties as assigned  

 

Requirements: 

• Active New York State (NYS) RN License 

• Home Health Care experience preferred 

• Valid driver's license and reliable transportation
