Overview of Position:

Provides general clinical pharmacy services, which include D.U.E. development and reporting, some teaching opportunities, as well as administration of research protocols. Compounds and/or dispenses medications to patients and provides drug information and education to all customers as to optimize the quality of care.

ESSENTIAL DUTIES
1.    Assists in orienting new personnel or students to department services and programs so they are made aware of their specific job requirements.
2.    Assists in providing educational in service programs in order to share the latest treatments and therapies with other care providers.
3.    Contacts prescribers to clarify medication orders or notify them of medications that are unobtainable to reduce the possibility of medication errors or delay in therapy.
4.    Assists in the inventory and ordering of pharmaceuticals so that stock outages are minimized and appropriate inventory levels are maintained.
5.    Helps evaluate processes that utilize material resources in order to provide cost effective care within budget.
6.    Assists the Purchasing Associate in procuring quality generic products as well as supports the formulary practices of the department to provide optimal care at the most affordable price.
7.    Maintains medication on assigned patient care areas by initialing inspection forms and submitting reports to the manager.
8.    Promptly prepares emergency carts and boxes to assure adequate quantities of in-date medication are available for emergency situations.
9.    Calculates caloric and/or electrolyte ingredients for TPN resulting in correct administration of patients’ nutritional therapy.
10.    Provides drug and toxicological information to customers in a timely and accurate fashion in order to maximize knowledge and minimize risk/adverse events.
11.    Accurately performs medication order entry on computer system, assuring completeness of the patient’s medication profile.
12.    Monitors drug therapy of patients by screening for potential medication problems so that issues involving drug interactions, incompatibilities, and potential allergies are resolved without patient harm.
13.    Applies pharmacokinetic/pharmacodynamic principles to individualize drug dosing for patients.
14.    Responds to and supports Code Blue and Code Trauma situations to maximize opportunity for patient survival and stabilization.
15.    Appropriately adjusts doses in patients with diminished renal or hepatic function in order to avoid potential toxicities by patients receiving too much drug.
16.    Individualizes the dose of medication for neonatal, pediatric and geriatric patients to optimize care during this phase of human development.
17.    Calculates appropriate doses and compounds chemotherapeutic agents in order to care for out cancer patients.
18.    Recommends to management formulary positions/approaches that would reduce costs and maintain/improve pharmaceutical care.
19.    Is familiar with spread-sheet and clinical based computer programs to enhance our manipulation of data to better compete in a managed care environment.
20.    Assists management in the administration of approved investigational drug research protocols in cases where it supports our mission by allowing patients therapeutic opportunities/options apart from conventional care.

QUALIFICATIONS
1.    Licensed Pharmacist in the State of Ohio.
2.    Pharmacy doctorate (PharmD) or equivalent.
3.    At least two years experience in hospital or similar health system.
4.    Excellent interpersonal, decision-making, facilitation, conflict resolution and investigative skills.
5.    Excellent listening, verbal and written communication skills.
6.    Ability to provide outstanding customer service and maintain a high degree of confidentiality. 
7.    Ability to work independently and under pressure in a complex and changing working environment.
