The Pediatric Complex Care Nurse Practitioner (NP) works in collaboration with a multidisciplinary health care team. The Complex Care NP provides direct medical care, telemedicine, care coordination, phone management, and on-call coverage for patients in the Program for Children with Medical Complexity under the supervision of Pediatric Complex Care Attendings. The NP has didactic education, clinical competency, and national certification in identified area of clinical practice. The NP demonstrates medical knowledge, clinical judgment, technical competence, professionalism, interpersonal communication skills, timely and compliant documentation and billing, and is responsible for professional development and competency validation. The NP has an advanced knowledge of nursing theory and application. The NP reviews and documents history, evaluates the current clinical status, summarizes the clinical findings in comprehensive plan-of-care, places appropriate orders, and collaborates with the attending physician for the plan of care. Using a patient centered approach to preventative and acute management, the NP positively impacts the course of the complex child by forming a comprehensive plan of care, preventing ER visits and hospitalizations, and providing care coordination and case management services. The NP bills for time spent and medical complexity for all clinical time as appropriate..
- Obtains and documents interim health history; performs and documents complete physical examination, assessment and comprehensive plan of care.
- Collaborates with multiple subspecialities to formulate and document an up-to-date plan-of-care to be used by the children’s hospital for each patient.
- Orders, performs, interprets, and collects data using appropriate assessment techniques, relevant supporting diagnostic information and diagnostic procedures where indicated.
- Forms differential diagnoses and treats acute and chronic conditions both in-person and via telemedicine.
- Prescribes therapeutic interventions both pharmacologic, non-pharmacologic and surgical, needed to achieve expected outcomes.
- Utilizes evidence-based, approved practice protocols in planning and implementing care;
- Initiates appropriate referrals and consultations; works with community agencies, insurers, and ancillary service providers to navigate barriers to care; coordinates service provision within the medical center. Triages to appropriate care venues and performs case management as appropriate between in-person services.
- Provides specialty specific consultation services upon request and within specialty scope of practice; performs on-call services for triage of patients outside normal business hours
- Facilitates the patient’s transition to adult setting as appropriate.
