A Brief Overview
The Nursing Assistant (NA) at Stanford Health Care functions under the direction and supervision of a Registered Nurse. The NA supports the care team through implementation of direct and indirect patient care. They record and report patient information, provide assistance with activities of daily living, and perform simple treatments and procedures. The NA allows for the provision of optimal patient care by ensuring a clean, orderly patient and unit environment and communicating patient status changes to the relevant RN or Resource Nurse.

• Essential Functions: The essential functions comprise of, but are not limited to:
• o Providing appropriate, direct and indirect patient care.
• o Assisting patients with activities of daily living.
• o Maintaining an accurate record of patient information and procedures performed.
• o Ensuring patient and unit environment cleanliness and order.
• o Responding appropriately to emergent events.
• o Ensuring patient identification and allergy band presence on the patient at all times.
• o Carrying out other clerical tasks as needed.
• Safety: Maintain a clean, safe working environment:
• o Cleaning and disinfecting equipment as appropriate
• o Maintain unit equipment and supplies in a manner consistent with regulatory requirements
• o Maintain clear and uncluttered paths of travel and egress for patients, employees, and visitors
• o Identify and escalate defective/broken equipment and supplies
• o Contributing to quality/process improvement and unit-based initiatives when appropriate
• Responsible for the strict maintenance of patient confidentiality and compliance with facility and regulatory agency requirements.
• May cross train to the Unit Secretary role to assist in providing additional Unit Secretary coverage as appropriate per the manager’s discretion.
• Adheres to the standards of nursing care and the policies and procedures of the Hospital and Department of Patient Care Services.
• Documents vital signs on stable patients requiring vital signs. Stable is defined as patients that require vital signs no more often than every 4 hours. The initial set of admission vital signs has been completed by the RN.
• Completes orientation program and participates in learning activities to maintain and improve knowledge skills.
• Considers age-specific needs when interacting with patients and families. Effectively communicates and interacts with patients, families, staff and members of the community from diverse backgrounds.
• Prioritizes patient care and care related activities.
• Demonstrates effective communication skills, especially in stressful situations. Communicates in a compassionate manner, is patient and courteous utilizing active listening skills as well as helping to maintain a quiet patient care environment.
• Dresses appropriately and wears name badge.
• Follows unit and departmental guidelines relate to staffing and scheduling.
• May provide break relief or assistance to the unit clerk as needed and if applicable.
• On certain units, after cross-training in the area of rehabilitation activities, assists patients with and/or provides efficient care related to rehabilitative activities as delegated and supervised by a Physical, Occupational, or Speech Therapist.
• Orients patient and family to environment.
• Performs routine patient care (hygiene, nutrition, elimination, measuring I/O's) utilizing universal precautions under the direction and supervision of a RN as assigned. Note: On selected units, with Patient Care Manager approval, may perform specific skills after documented instruction, such as electrocardiograms.
