School-Based Registered Nurse (SBT)

Constellation Kids is looking to add a School-Based Registered Nurse RN to our team who will work collaboratively with the school team and other clinicians to provide services to children in a school setting. They will also provide documentation and reports at IEP meetings.

The Role:

· In sync with the school calendar.

· Provide skilled nursing care for students with complex medical needs in the public school setting.

· Follow students’ health care plan or medical recommendations within the routine of the school day.

· Collaboration with other disciplines including special education teachers, related service providers and paraprofessionals to ensure that medical interventions/support are implemented appropriately throughout the school day across settings.

· Develop, monitor, revise students’ health care plan if needed.

· Support students on field trips or on the school bus, if required.

· Attend PPTs as directed.

· Communication and coordination with families.

· Supervision of CNAs.

Qualifications:

· Connecticut State Licensed Registered Nurse.

· Completion of necessary nursing qualifications from an accredited school/program.

· CT Department of Health License/certification.

· Pediatric experience preferred.

· School-based experience preferred.

· Strong oral and written communication skills.
