- Part-Time (0.8FTE) & Full-Time position available + Hybrid working conditions
- 12-month maternity backfill position
- Work from our Ferntree Gully office (Woiwurrung, Boon Wurrung & Bunurong Nations) + Travel when required
- $51.76 per hour + Generous Salary Packaging benefits of up to $18,550
What you’ll be doing
Are you ready to make a real difference in the lives of Older Adult Clients? In this dynamic role, you’ll work hand-in-hand with customers and their support networks to empower them to maintain their independence in a way that aligns with their unique preferences.
You’ll focus on creating opportunities for equitable outcomes for our clients by:
- Transform Lives: Conduct initial and ongoing assessments and care planning to support customers at home.
- Be a Health Advocate: Monitor health needs and provide guidance, coordinating with agencies and healthcare professionals.
- Drive Service Excellence: Manage service delivery within individual plans and budgets.
- Collaborate for Success: Partner with customers and their support networks to review and adjust plans, ensuring their needs are always met.

Who we’re looking for
If you're an experienced Case Manager, qualified as a Nurse, who is confident working across multiple IT systems, and capable of effectively engaging individuals and families to build positive and sustainable relationships, then you might be the perfect person for our team!
Experience and skills require:
- Experience providing clinical care in home, community, or aged care settings for older adults and their caregivers.
- Desired experience in Home Care Package delivery.
- Advanced skills in assessment and clinical care planning to promote independent living.
- Ability to evaluate and ensure high-quality, effective clinical care services.
- Proficient in information technology.
- Demonstrated experience managing brokerage budgets, including invoicing and financial processing, with client negotiations.
- High sensitivity to the issues affecting older adults and a strong commitment to advocacy.
Qualifications
- Bachelor of Nursing or equivalent + Current Registered Nurse for a Clinical position.
- Current state-based driver’s license.
