New York Health is seeking a dedicated and compassionate Advanced Practice Provider to join our primary care team in Bay Shore, New York. In this role, you will deliver patient-centered care through evaluations, follow-up visits, and treatment planning in close collaboration with experienced physicians. Salary Transparency:

Base Salary Range: up to $165,000 annually
