We are searching for a talented RN-Medical Care Management
Hours: 40hrs
Shift: Day/Evening shift; 8:00am - 4:30pm
Location: Southcoast Business Center – Fairhaven, MA; This position will follow a hybrid schedule
Travel: Driving on average 3 or more times per week required

Responsibilities
Under direction of the manager, care management, performs a wide variety of duties that primarily focuses on supporting effective patient care and positive outcomes across the transition of care continuum for each patient identified for care management. Position responsibilities include: assessing and identifying learning needs for this population of medically complex patients, working with this designated group of patients and their community providers to reduce hospital visits or decrease 30-day readmission rates by addressing barriers and opportunities to better support healthy outcomes & behaviors.Qualifications
- A Bachelors degree in Nursing or the equivalent education and experience is required.
- Current RN Registration in the Commonwealth of Massachusetts and CPR certification is required.
- Three (3) years of clinical experience with at least 2 years in an acute care setting is required.
- RN licensure in State of RI within 6 months of employment.
- OB/gyn experience necessary and multilingual preferred
Must be fully vaccinated against seasonal Influenza and the COVID 19 virus or to be exempt from the requirement for medical or personal reasons by signing a statement certifying you are choosing to be exempt from vaccination once hired.
Compensation: Pay rate will be determined based on level of experience.
Pay Range
