What we are looking for:
- Graduate of an accredited school of professional nursing; BSN required.
- BSN in nursing and two years combined public health nursing and progressive professional responsibilities in public health nursing.
- Current license in good standing as a Registered Nurse in the state of New Jersey.
- Minimum of two (2) years nursing experience in an acute hospital setting or equivalent experience with one year of supervisory experience.
- Demonstrated ability to supervise and direct clinical and administrative personnel.
- Complies with accepted professional standards and practice.
- Excellent verbal and written communication skills and strong interpersonal skills.
- Knowledge of home care federal and state regulations.
- Knowledge of reimbursement sources and documentation requirements within home care.
- Must be a licensed driver with automobile insured in accordance with Agency and state requirements.
- Has current CPR certification.
What you will be doing:
- Provide direct, continuous skilled nursing care to adult patients in the home.
- Perform and document comprehensive nursing assessments and ongoing reassessments.
- Implement and follow an individualized Plan of Care (POC) approved by the physician.
- Administer medications, treatments, and therapies per physician orders.
- Monitor and manage complex medical conditions (e.g., tracheostomy, ventilator, G-tube, seizure management).
- Maintain accurate, timely, and compliant clinical documentation in accordance with ACHC and NJ regulations.
- Educate and support patients and certified home health aides (CHHA) regarding care plans and treatments.
- Communicate effectively with physicians, care managers, and interdisciplinary team members.
- Participate in case conferences, supervisory visits, and quality improvement activities.
- Ensure care is delivered in accordance with infection control and patient safety standards.
Hourly Range:
- 32.00-40.00 per hour based on years of experience.
