Position: Full-Time; Hybrid Role - 3 Days in Clinic / 2 Days Remote

JOB SUMMARY
The Oncology Nurse Navigator will support our new RAPID clinic. RAPID Clinic- Rapid Assessment for Potential Incidental or Diagnosed malignancy. The Nurse Navigator calls patients within 24 hours, develops relationships and assess for social determinants of health barriers. Ensures all records, documentation, etc. are available for patient's first visit. Navigate patient to diagnosis with hand offs to care team.
This job will assist the patient, family and care giver to overcome healthcare barriers (including but not limited to social, financial, or physical barriers.) Responsible for conducting on-going assessments of need and facilitate referrals to additional Support Services throughout the Oncology continuum of care. Using the nursing process and evidence based knowledge, the Oncology Nurse Navigator provides education and resources to facilitate informed decision making (by the patient and/or family.)

EXPERIENCE AND EDUCATION
Required
- Education
Associate's Degree Graduate of an approved/accredited nursing program either by a governmental authority, nursing board, or by a licensing authority that has jurisdiction over approval/accreditation of nursing programs (Texas BON 217.1)
- Experience
3 years oncology nursing experience
- Licenses and Certifications
(RN) REGISTERED NURSE and
(BLS) BASIC LIFE SUPPORT course accredited by the American Heart Association (AHA) or American Red Cross (ARC), are required. and
Preferred
- Education
Bachelor's Degree Bachelor of Science in Nursing.
- Licenses and Certifications
(OCN) ONCOLOGY CERTIFIED NURSE or
(CPHON) CERT PED HEMATLGY ONCOLGY NURS or
(CBCN) CERTIFIED BREAST CARE NURSE or
(BMTCN) BloodMarrow Transplant Cert RN
