As an RN on the Home Care Team, you will deliver the best possible care to patients in the comfort of their own homes within the West of I-77 Service area. In this role, you will provide assessments, complex treatments and education to help avoid readmissions to the acute care setting and ensure comfort. This position offers a flexible schedule, allowing you to maintain a healthy work-life balance, all while changing the lives of your patients and their loved ones every day. 

A caregiver in this position works varying shifts, with 48 hours/six-week period, weekend and holiday requirements. Orientation is 8 weeks, Monday to Friday, 8a-5p.

A caregiver who excels in this role will:  

  • Provide direct nursing care in accordance with established healthcare organization policies, procedures, and protocols. 
  • Implement and monitor patient care plans.  
  • Provide supervision to LPNs, Home Care Aides and families/caregivers involved in the care of the patient. 
  • Perform accurate and complete admission assessments and reassessments within a specified time frame. 
  • Assess patient for safety risks, abuse and neglect. 
  • Serve as a primary coordinator of all disciplines for well-coordinated patient care. 
  • Assess and document patients' pain using appropriate pain scale and manage patients' pain then. 
  • Prioritize and document patient problems based on assessed needs and problems in collaboration with patients, family and healthcare team. 
  • Maintain complete and up-to-date documentation and files at all times regarding patient condition and effectiveness and timeliness of care and interventions. 
  • Escalate change in patient condition to patient’s physician as appropriate. 

Minimum qualifications for the ideal future caregiver include:  

  • Graduate from an accredited school of Professional Nursing 
  • Current state licensure as a Registered Nurse (RN) 
  • Basic Life Support (BLS) Certification through the American Heart Association (AHA) 
  • One year of clinical experience as an RN in an acute medical/surgical setting 
  • Six months of experience as a Homecare Nurse 
  • Valid Driver’s License (VDL) in Ohio and proof of automobile insurance with $100,000/$300,000 coverage 

Preferred qualifications for the ideal future caregiver include: 

  • Bachelor’s of Science in Nursing (BSN) 
  • Infusion therapy experience
