Licensed Practical Nurse - FT - Days (73936)

Hours: 8AM - 5PM, with occasional on call duties

Days: Monday - Friday

JOB SUMMARY

Under the direction and supervision of the Home Health Care RN Case Manager provides skilled nursing care according to the Patient’s Plan of Care. Duties at times may include various office duties such as chart review, and back up for the Intake Coordinator. Other duties may include coverage for Home Health Aides when needed during crisis times and frequencies must be met to meet regulatory compliance.

This position is responsible for assisting in the implementation of the patient's care plan and assisting the patient in maintaining good personal hygiene and a safe and clean environment.

JOB QUALIFICATIONS

Education: A graduate of an accredited school of nursing Practical, Diploma, Associate, Baccalaureate, or masters preferred.

Licensure: Current Georgia licenses as a LPN. Must have a current Georgia drivers license and valid automobile insurance coverage.

Experience: 2 years as a Home Health LPN

Skills: Nursing skills as defined in the Law Governing the Practice of Practical Nursing for LPNs in Georgia. Requires interpersonal skills, ability to learn admission criteria, coordination of services, and community resources and intermediary requirements. Demonstration of ability to complete the paperwork involved in the recertification process and an understanding of Home Health concepts and regulatory requirements. Knowledge of common safety hazards and precautions for a safe work environment. Skill in modifying documentation to meet intermediary requirements. Skill in preparing, maintaining and writing reports. Ability to maintain effective working relationships with associates, patients, medical staff and the general public. Ability to interpret, adapt, and apply guidelines and procedures. Ability to communicate clearly.
