We began our partnership with Northside Gwinnett in 2013 to provide therapy services throughout the Northside Gwinnett system. Innovative Therapy Concepts is searching for a Registered Nurse to work in our Cardiopulmonary Rehab Department. A minimum of twelve months of experience with the treatment of cardiovascular patients is necessary. The Cardiac Rehab RN specializes in the care and management of pre and post-cardiac care of adult and geriatric patients; provides rehabilitative care, education, and psychosocial support to patients; and acts as a resource for nursing personnel. The Cardiac Rehab RN will work both in the acute and outpatient settings providing care to Cardiac patients receiving Phase 1, 2 and 3 Rehabilitation. Other important qualities we are searching for are teamwork, excellent communication, ability to multi-task and flexibility.

Responsibilities:

  • Demonstrates competency in clinical practice; consistently utilizes the nursing process to identify normal and abnormal findings and changes from baseline.
  • Provides safe, competent patient care; is knowledgeable of current policies and procedures that impact the specific patient population.
  • Knowledgeable of pathophysiologic states common to the unit/service patient population.
  • Effectively communicates with peers, colleagues, providers and others regarding patient condition and subsequent changes.
  • Independently implements the nursing process for patients and families; learns and utilizes unit- and service-wide evidence-based practice resources for clinical practice.
  • Utilizes patient care and organizational technology specific to the unit, patient population and/or care setting is safely accomplished.
  • Successfully interfaces/operates the organization’s computerized information systems; continuously develops personal practice competencies, skills and knowledge.
  • Delegates care as appropriate.
  • Serves as a preceptor for new staff members and students as needed.
  • Creates personal goals for lifelong learning and professional growth.
  • Conducts self-assessment of performance and discusses with leadership.

  • Collaborates with unit leadership, peers and colleagues to identify own learning needs; collaborates on performance assessments of others.
  • Recognizes ethical issues in practice; organizational resources to access in ethical dilemmas are known. Performs nursing actions in response to a patient/family need for advocacy.
  • Communicates and consults with other health care providers to ensure continuity of care.
  • Uses evidence-based guidelines and policies to guide professional practice.
  • Seeks out new knowledge and expanded skills/competencies to improve performance.
  • Expands knowledge of the unit’s/service’s Quality Measures; uses quality improvement findings/scores to guide and guide practice.
  • Participates in established quality improvement studies and/or activities for patient safety and quality measures; participates in continual accreditation readiness activities.
