Mass General BrighamAbout the role
Mass General Brigham relies on a wide range of professionals, including doctors, nurses, business people, tech experts, researchers, and systems analysts to advance our mission. As a not-for-profit, we support patient care, research, teaching, and community service, striving to provide exceptional care. We believe that high-performing teams drive groundbreaking medical discoveries and invite all applicants to join us and experience what it means to be part of Mass General Brigham.
Our Community Health Workers (CHWs) are integral and trusted members of the community helping our high-risk patients who are getting care at MGB primary care practices receive the services they need to achieve an optimum quality of life. This high-risk patient population presents a unique set of challenges, including higher rates of mental illness and barriers to care, and are often harder to contact and engage, as well as having weaker ties to their primary care practice. The CHW acts as a bridge between our primary care teams and our high-risk patients, improving their access to and utilization of healthcare services.
