Position Overview
The Patient Management Pharmacist plays a critical role in ensuring accurate, timely, and effective drug therapy for patients while providing expert guidance on medication use and management. This position supports day-to-day pharmacy operations, performs clinical reviews, manages drug inventory, and collaborates closely with physicians, nurses, and other healthcare professionals to deliver high-quality pharmaceutical care. The Patient Management Pharmacist also provides education, drug utilization review, and supports initiatives aimed at improving medication adherence and patient outcomes.

Key Responsibilities

• Manage daily pharmacy operations to ensure consistent delivery of patient-centered pharmaceutical services.
• Collaborate with infusion nurses, physician teams, and clinical staff to verify doses, assess drug interactions, and ensure medication accuracy.
• Maintain and monitor drug inventory, ordering supplies as needed to meet departmental goals.
• Maintain required logs and documentation to ensure compliance with regulatory and safety standards.
• Process claims, provide customer service, and serve as a liaison between patients and the care team.
• Support and cross-train across infusion pharmacy services as needed.
• Provide drug information and counseling for patients, caregivers, and providers regarding therapy, dosing, interactions, and new treatment options.
• Conduct medication therapy management and adherence programs with a focus on quality improvement.
• Perform discharge medication reviews, reconciliations, and patient counseling as appropriate.
• Evaluate physician treatment plans, ensuring therapeutic appropriateness of drugs, doses, and laboratory parameters.
• Provide oral medication education for all new West Cancer Center patients.
• Guide and mentor pharmacy technicians, students, and trainees in assigned work areas.
• Participate in staff meetings, departmental committees, and workgroups to improve clinical processes and promote best practices.
• Uphold patient confidentiality, professional conduct, and adherence to all clinic safety and compliance policies.

Qualifications

Education & Experience

• Required:
• Bachelor’s degree in Pharmacy.
• Completion of Oncology Pharmacy curriculum within 30 days of hire (if not BCOP certified).
• 1–2 years of related experience or 5–7 years of related experience and/or training or an equivalent combination of education and experience.
• Preferred:
• Doctor of Pharmacy (Pharm.D.) degree.

Credential/Licensure

• Required:
• Licensed or eligible for licensure as a pharmacist in the state of Tennessee.
• Preferred:
• Board Certified Oncology Pharmacist (BCOP) credential.

Skills & Abilities

• Strong knowledge of oncology drug therapy, pharmacology, and medication management.
• Excellent verbal and written communication skills, with the ability to collaborate effectively across multidisciplinary teams.
• Strong organizational, analytical, and problem-solving skills.
• Proficiency in computer applications, including word processing, email, and EMR systems.
• Ability to manage multiple priorities in a fast-paced environment.
• Commitment to professionalism, teamwork, and continuous learning.
• Ability to lift up to 50 lbs. and meet physical demands typical of a clinical environment.
