Position Summary

The Physician Assistant (PA) in the Post-Acute Care setting provides high-quality, patient-centered medical care to individuals transitioning from acute hospital care to skilled nursing or rehabilitation settings. The NP works collaboratively with physicians, nursing staff, and interdisciplinary teams to optimize patient outcomes, ensure continuity of care, and support safe discharges

 

Position Responsibilities

  • Conduct comprehensive history and physical examinations on post-acute and rehab patients.
  • Develop, implement, and evaluate care plans in collaboration with interdisciplinary teams.
  • Manage chronic and acute medical conditions, including medication management, labs and diagnostic interpretation.
  • Round on patients regularly to monitor progress, address new or ongoing concerns, and support rehab goals.
  • Facilitate care transitions by communicating with hospitalists, primary care providers, specialists, and family members.
  • Document all care provided in the electronic health record (EHR) in a timely and accurate manner.
  • Participate in quality improvement initiatives, care conferences, and regulatory compliance efforts.
  • Educate patients and families on diagnoses, treatments, medications, and discharge plans.
  • Provide end-of-life care planning and palliative support when appropriate.
  • Ensure adherence to clinical guidelines, facility policies, and state/federal regulations.

 

Position Experience Required

Minimum of 1-2 years of clinical experience preferred, ideally in geriatrics, internal medicine, or post-acute/long-term care

Ability to work both independently and collaboratively within a healthcare team

Strong knowledge of SNF regulations, transitional care, and rehabilitation principles

-Master’s degree from an accredited Physician Assistant program.

-Current certification by the National Commission on Certification of Physician Assistants (NCCPA).

-Active and unrestricted PA license in NJ

-DEA & NJ Controlled Dangerous Substance (CDS) license  

-BLS certification
